DMP-Disease Management Programme

Disease management programs (DMP) are structured treatment programs that are intended to help chronically ill people get their illness under control and improve or maintain quality of life. Last but not least, DMP was introduced with the aim of improving drug treatment in the long term. These programs are also called “chronic programs”.

In our practice we offer the following programs:

  • DMP for diabetes mellitus type 2
  • DMP for COPD (Chronic Obstructive Pulmonary Disease)
  • DMP for Asthma
  • DMP for CHD (coronary heart disease)
  • DMP for osteoporosis

Close checks every 3 months have proven to significantly reduce the complications of these diseases.
